Job summary

Pride Health is hiring a Physical Therapist to support our client's healthcare facility in Madison, OH. Schedule: Monday–Friday, 8:00 AM–4:30 PM. Location: Madison, OH.

Local contract for 13 weeks with a pay range of $50.00–$55.00 per hour and mileage reimbursement at $0.49 per mile, when applicable. Duties include comprehensive home health evaluations, creating and adjusting treatment plans, delivering skilled PT interventions to improve mobility and function, educating patients and caregivers,

Qualifications

• DPT or equivalent accredited PT program.
• Active PT license - Ohio.
• BLS/CPR preferred.
• Home Health PT experience required.
• Home health documentation and EMR familiarity preferred.

Responsibilities

• Perform comprehensive physical therapy evaluations in the home setting.
• Develop and implement individualized treatment plans based on patient needs and goals.
• Provide skilled PT interventions to improve mobility, strength, balance, coordination, and functional independence.
• Monitor patient progress and modify treatment plans as clinically appropriate.
• Educate patients and caregivers on exercises, safety precautions, mobility techniques, and home programs.
• Coordinate patient care with physicians, nurses, therapists, and other members of the interdisciplinary team.
• Complete accurate and timely clinical documentation in accordance with home health and regulatory requirements.
• Manage an assigned caseload while meeting productivity expectations.
• Communicate changes in patient condition and progress to the appropriate care team.
• Maintain compliance with applicable clinical, safety, and documentation standards.
