Adaptive Home Health is seeking a Physical Therapist (PT) for field-based home health in Texas. You will own the caseload, perform in-home assessments, and deliver skilled PT visits focused on gait, transfers, strengthening, and home safety.

Qualifications

Active Texas PT license (in good standing).
Strong clinical judgment and ability to work independently in the field.
Comfortable escalating changes to RN/MD and documenting rationale clearly.
Strong communication and documentation skills.
Current driver’s license and willingness to travel daily across assigned territory.
Able to carry/transport basic therapy equipment as needed.
Competent in core home health PT scope: gait, transfers, balance, home safety.

Responsabilités

Own the caseload as a case manager: therapy plan, progression, and outcomes.
Complete comprehensive in-home assessments and deliver skilled PT visits.
Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.
Set measurable goals, track progress, and adjust treatment as patients stall.
Drive multidisciplinary coordination: communicate changes in condition, align on orders, keep the care team moving.
Manage treatment and outcomes directly, including PTA supervision where appropriate.
Coach patients and caregivers with practical home programs that actually get done.
Document promptly and thoroughly in Homecare Homebase (HCHB).
