Job summary

Adaptive Home Health, LLC is seeking a Physical Therapist to deliver skilled PT visits in patients' homes across the field-based territory in Texas. You will own the caseload, develop treatment plans, and coordinate with the RN/clinical team to ensure safe progression and outcomes, while leveraging technology to streamline charting and scheduling.

A Texas PT license and strong independent work ethic are essential.

Qualifications

• Active Texas PT license in good standing.
• Strong clinical judgment and independent field work.
• Excellent communication and documentation skills.
• Reliable follow-through on orders, frequencies, and goals.
• Willingness to travel daily within assigned territory.

Responsibilities

• Own the caseload, develop progression and outcomes.
• Perform comprehensive in-home assessments and deliver skilled PT visits.
• Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety.
• Coordinate with RN/MD and keep the care team aligned; document clearly in HCHB.
• Coach patients and caregivers with practical home programs that actually get done.
