Job summary

Elara Caring in Louisiana is seeking a dedicated Physical Therapist to deliver home-based care. You will evaluate patients, develop individualized treatment plans, monitor progress, and adjust plans as needed, coordinating with physicians and the care team.

Requirements include a degree from an APTA-approved PT program, an active PT license for the state, at least one year of healthcare experience, and a valid driver's license or reliable transportation.

Qualifications

• Graduate of an APTA approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one (1) year of experience in a healthcare facility.
• Must possess a valid driver's license or reliable transportation to travel between patient homes and work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in the home setting.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation.
• Supervise therapy assistants and support staff, as appropriate.
