Elara Caring seeks a dedicated Physical Therapist to join our team delivering top-tier home-based care. You will evaluate patients, create personalized therapy plans, and provide services directly in patients' homes, collaborating with physicians and caregivers to ensure seamless care.

The role requires a PT license in the state of work, at least one year in a healthcare setting, and reliable transportation to travel between patient homes as needed.

Qualifikationen

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one (1) year of experience in a healthcare facility.
• Must possess a valid driver’s license, maintain an acceptable driving record, or have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.

Aufgaben

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in the home setting.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation.
• Supervise therapy assistants and support staff, as appropriate.
