Job summary

Elara Caring in Louisiana seeks a dedicated Physical Therapist to join our home care team, providing high-quality care to patients in their homes. You will evaluate patients, develop treatment plans, and collaborate with physicians and caregivers to adjust care as needed.

The role requires an APTA-approved program graduate, an unrestricted PT license for the state, and at least one year of healthcare facility experience. A valid driver's license or reliable transportation is required.

Qualifications

• Graduate of an APTA-approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Must possess a valid driver's license or have reliable transportation to travel between patient homes and locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in the home setting.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation.
• Supervise therapy assistants and support staff as appropriate.
