Job summary

Elara Caring seeks a dedicated Physical Therapist to deliver high-quality care in patients’ homes across Louisiana. You will evaluate patients, develop tailored treatment plans, and collaborate with the care team to ensure optimal outcomes.

Requirements include an APTA-approved PT program, an active PT license in the state, and at least 1 year in a healthcare setting. Travel between homes is required, with a valid driver's license or reliable transportation.

Qualifications

• Graduate of an APTA-approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Must possess a valid driver’s license or have reliable transportation to travel between patient homes and work locations.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in the home setting.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation.
• Supervise therapy assistants and support staff, as appropriate.
