Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Assists physician in development of therapy plan of care.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient response to treatment and changes in condition.
• Treats patients to relieve pain and restore function.
• Instructs patient and family regarding plan of care, functional limitations, and progress.
• Completes progress notes for visits and submits per policy.
• Develops PT plan of care within state guidelines.
• Reports outcomes of evaluation and goals.
