Job summary

Enhabit Home Health & Hospice is seeking a Physical Therapist to provide in-home therapy services, assess mobility needs, and develop treatment plans for patients recovering at home. You will work closely with physicians, families, and care teams to support independence and functional goals.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments to improve mobility and function in the home setting.
• Administer skilled care to clients under physician orders and established plan of care.
• Instruct family and other health care team members to support therapy goals.
• Operate under the direction of the Branch Director and collaborate with the care team to ensure quality outcomes.
