Job summary

Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to provide skilled home health services, assessing mobility limitations and delivering interventions that restore function and independence in daily activities.

You will collaborate with physicians, caregivers, and the health care team to implement individualized plans of care in patients' homes, while educating families on safe, effective treatment and ongoing self-management.

Qualifications

• Must possess a valid state driver's license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments to improve mobility and function.
• Administer skilled care to clients under physician orders and plan of care, in home settings.
• Educate family and health care team members to support ongoing self-care and safety.
