The role requires a Master's in Physical Therapy, current Texas PT license, CPR certification, and at least 9 months of clinical experience, preferably in home health. Travel to patient locations is required, and the position offers full-time hours with salary based on experience.

Qualifications

• Master's degree in physical therapy from a CAPTE-accredited program.
• Minimum nine months of PT experience.
• Preferred experience in community/home health.
• Licensed to practice as a PT in the state of operation.
• CPR certification required.
• Must be a licensed driver able to travel to all business locations.

Responsibilities

• Provides patient evaluation and treatment per physician orders and agency policy.
• Maintains documentation and tracks patient progress.
• Educates patients and families about treatment plans and self-care.
• Develops and documents treatment plans and priorities.
• Delivers organized, efficient patient care and documents accordingly.
• Communicates patient status and treatment plans to the medical team.
• Treats patients within agency productivity standards and payer guidelines.
