AccentCare in College Station, TX is seeking a Physical Therapist for Home Health on a PRN basis. You will evaluate and treat patients in their homes, collaborating with a multidisciplinary team to deliver high-quality care and patient education.

Qualified candidates hold a CAPTE-approved Master’s in Physical Therapy, have at least nine months of PT experience, and possess a current Texas license and CPR certification. A valid driver’s license is required for local travel to agency locations.

Qualifications

• Master’s degree in physical therapy approved by CAPTE accredited program.
• Minimum nine months experience as a physical therapist.
• Licensed to practice as a physical therapist in the state of operation.
• CPR certification current.
• Must be a licensed driver able to travel to all locations.

Responsibilities

• Provide evaluation and treatment to patients per physician orders and agency policy.
• Perform patient evaluations and select diagnostic materials.
• Inform physician of assessment results and recommended interventions.
• Develop and document treatment plans with clear goals and priorities.
• Educate and counsel patients and families; document progress.
• Communicate patient status and plans with the team and physicians.
• Meet productivity standards and manage caseload efficiently.
