Qualifications

• Current license to practice physical therapy in the state assigned (GA).
• Graduate from CAPTE-accredited PT program.
• At least 1 year of related PT experience in home health care; mentoring if less than 1 year.
• Current CPR certification.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions and conduct reexaminations every 30 days.
• Modify interventions as needed and perform discharge planning.
• Other duties as assigned.
