Job summary

CHRISTUS Good Shepherd HomeCare, part of the LHC Group and Optum family, is hiring a Physical Therapist in Home Health to perform assessments, develop treatment plans, and deliver interventions that enhance patients' well-being in home settings.

You will work with a care team, visit patients per the plan of care, document progress, and ensure compliance with state guidelines. Requires active PT license and strong communication; valid driver's license a must.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and access to a dependable vehicle or reliable transportation.
• CPR certification or ability to obtain within 90 days of hire.
• Home health or post-acute care experience is a plus.

Responsibilities

• Evaluates the patient’s functional status and therapy needs with physician collaboration.
• Develops and follows the plan of care.
• Provides PT services within scope of practice and plan of care.
• Monitors patient response and reports changes to supervising nurse/physician.
• Treats to relieve pain and restore function according to standards.
• Educates patient and family about plan of care and progress.
• Visits patients per Plan of Treatment and documents progress notes.
• Develops PT plan of care within state guidelines and reports outcomes.
• Functions in diverse home settings regardless of demographics.
