CHRISTUS Good Shepherd HomeCare - Longview is seeking a Physical Therapist to evaluate patients in home health, develop treatment plans, and deliver interventions to improve mobility and quality of life. You will document progress and collaborate with the care team to ensure effective, patient-centered care.

The role requires current PT licensure in the state, a valid driver's license/vehicle access, and CPR certification (preferred within 90 days).

Competenze

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Mansioni

• Evaluates the patient's functional status and physical therapy needs; assists the physician in the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and plan of care; collaborates with health care team.
• Observes and reports patient response to treatment to supervising nurse/physician and updates plan accordingly.
• Treats patients to relieve pain, restore function, and maintain maximum functioning per standards of practice.
• Instructs patient, family, and care team about plan of care, limitations, and progress toward goals.
• Visits patients per Plan of Treatment; completes and submits progress notes per policy.
• Completes evaluations and develops PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and projected frequency of care.
• Ability to work with diverse patients in various home settings.
