Job summary

UnitedHealth Group's Mountaineer Home Care, part of LHC Group, seeks a Physical Therapist in Home Health to assess patient needs, develop treatment plans, and implement interventions in Kanawha County. You will evaluate mobility, endurance, pain, and functional status to improve well-being and progress toward goals.

This role requires current PT licensure in the state, a valid driver's license with vehicle access, and CPR certification or the ability to obtain within 90 days of hire.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and access to a vehicle or public transportation.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and assists in developing the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and in coordination with the health care team.
• Observes and reports patient responses to treatment and any changes to supervising staff.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Informs patient and family about plan of care, progression and functional limitations.
• Visits patients according to Plan of Treatment and submits progress notes per policy.
• Completes all evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals and anticipated frequency of care.
• Works effectively with patients from diverse backgrounds.
