Job summary

AMEDISYS HOME HEALTH is seeking a Physical Therapist to provide goal-oriented care in a home-health setting. This full-time role offers daytime hours and a chance to help patients improve movement and function in the comfort of their homes.

The ideal candidate holds a state PT license, CAPTE-accredited education, and CPR certification, with at least one year of PT experience in home health or readiness with regional mentoring if less than one year.

Qualifications

• Current license to practice physical therapy in the state you’re assigned to work in.
• Graduate from an CAPTE-accredited physical therapy educational program.
• One year related physical therapy experience for home health care.
• If less than one year of experience, approval from regional clinical leaders and mentoring/ residency may be required.
• Current CPR certification.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provides interventions, conducts reexaminations every 30 days, modifies interventions as necessary, and performs discharge planning.
• Other duties as assigned.
• Collaborate with the care team to coordinate services and ensure patient progress.

• $115,000.00 - $127,900.00

Qualifications

• Current license to practice physical therapy specific to the state you’re assigned to work in.
• Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).
• One year related physical therapy experience for PTs entering home health care to ensure that knowledge and skills are sufficient to safely provide physical therapy services to patients.
• If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participating in a residency program.
• Current CPR certification.
