Job summary

Amedisys, a leading home health and hospice provider, is seeking a Per Diem Home Health Physical Therapist to help patients improve movement and function in their homes.

Responsibilities include periodic reexaminations, adjusting interventions, discharge planning, and collaboration with care teams. Requirements: state PT license, CAPTE-accredited degree, CPR, and at least 1 year of home health PT experience or regional mentorship for less than a year.

Qualifications

• State license to practice physical therapy in the assigned state.
• Graduate from CAPTE-accredited PT program.
• At least 1 year of physical therapy experience in home health care; if less than 1 year, regional mentoring required.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Conduct reexaminations (every 30 days minimum), adjust interventions as needed, and perform discharge planning.
• Perform other duties as assigned.

• Provides interventions, conducts reexaminations (every 30 days, at minimum), modifies interventions as necessary, and performs discharge planning.
• Other duties as assigned.

Qualifications

• Current license to practice physical therapy specific to the state you're assigned to work in.
• Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).
• One year related physical therapy experience for PTs entering home health care to ensure that knowledge and skills are sufficient to safely provide physical therapy services to patients.
• If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participating in a residency program.
• Current CPR certification.
