Job summary

PruittHealth Home Health is seeking a Physical Therapist to evaluate and treat patients in their homes. The role features a new pay-per-point model, top industry pay, and a $10,000 sign-on bonus for qualified candidates.

PRN and local commuting to Madison County are emphasized to ensure continuity of care with the patient and family. The ideal candidate will hold a valid state PT license, be a graduate of an accredited PT program, and have home health experience preferred.

Qualifications

• Must have a valid professional license in the state of practice.
• Graduate from an accredited Physical Therapy program.
• If foreign-trained, must be deemed substantially equivalent by a credentials evaluation organization approved by APTA.

Responsibilities

• Evaluate function and skilled therapy needs to develop a plan of care.
• Treat to relieve pain and restore function, achieving maximum outcomes.
• Consult family and interdisciplinary team to promote optimal outcomes.
• Document progress using required forms and within timelines.
• Collaborate with care team to develop and adjust the care plan.
• Maintain licensure, comply with safety regulations, and participate in quality initiatives.
