Job summary

PruittHealth at Home is seeking a Physical Therapist to provide evaluative, consultative and treatment interventions in patients' homes across Florence, Clarendon, and Sumter counties.

You will develop and implement plans of care, coordinate with physicians and families, document outcomes, and maintain licensure. The role offers full-time hours, a new pay per point compensation model, mileage reimbursement, paid time off, and a supportive team environment.

Qualifications

• Must be a licensed Physical Therapist with appropriate state licensure.
• Graduate from an accredited Physical Therapy program.
• Foreign-trained applicants must have credentials evaluated as equivalent by APTA-approved organization.

Responsibilities

• Evaluate patient function and skilled therapy needs.
• Treat to relieve pain, restore function, and achieve outcomes per plan of care.
• Advise families and team members to promote optimal functional outcomes.
• Document patient progress using required forms and timelines.
