Job summary

PruittHealth at Home is seeking a PRN Physical Therapist to provide evaluative, consultative and treatment interventions to home health patients in their residences.

New pay per point model offers top pay and incentives; must hold a valid state license and graduate of an accredited PT program. Experience in home health is preferred; willingness to travel within Union County, NC is a plus.

Qualifications

• Must hold a valid physical therapy license in the practicing state.
• Graduate of an accredited Physical Therapy program.
• If foreign-trained, program must be substantially equivalent per APTA.
• Home health experience is preferred.

Responsibilities

• Evaluate the patient’s level of function and skilled therapy needs.
• Treat the patient to relieve pain, develop or restore function, and obtain maximum outcomes according to the established plan of care.
• Advise and consult the family and other interdisciplinary team members to promote optimal functional outcomes.
• Assesses, monitors, documents and reports progress of patient’s health and condition using required documents via clinician preferred technology, and within established timelines.
• Maintains current license, certification, and clinical records to meet regulations.
