Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and needs; assists in development of the therapy plan of care.
• Provides services within the scope of practice and in coordination with the health care team.
• Observes and reports patient responses to treatment and changes in condition to supervising staff.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patients, families, and team members about plan of care and progress toward goals.
• Completes evaluations and documents progress notes per policy.
