Mederi Caretenders, part of the LHC Group, is hiring a Physical Therapist for Home Health. You will assess patients, develop plans of care, and provide interventions to improve mobility, endurance, and function in home settings.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient’s functional status and PT needs and assists in developing the therapy plan of care.
• Provides PT services within the scope of practice and plan of care in coordination with the health care team.
• Observes and reports patient response to treatment and changes in condition to supervisor/physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
