Atlantic Health is seeking a dynamic Physical Therapist to join the Visiting Nurse Department as a Full-Time role. The position involves evaluating and treating patients in their homes across Essex County, NJ, with work hours 8AM-4PM and one weekend every four to six weeks.

The ideal candidate will be part of a collaborative team of health providers delivering comprehensive, home-based physical therapy services.

Qualifications

• Licensed Physical Therapist in good standing
• Strong clinical assessment and treatment planning skills
• Excellent communication and teamwork abilities
• Experience delivering home-based PT services (preferred)

Responsabilités

• Perform initial examinations and differential diagnosis.
• Develop and implement plans of care with therapeutic exercises.
• Conduct manual therapies and modalities as indicated.
• Document care plans, progress notes, and billing information.
• Collaborate with interdisciplinary team to optimize patient outcomes.
