Job summary

Atlantic Visiting Nurse, part of Atlantic Health, is seeking a full-time Physical Therapist to join a collaborative home-health team serving patients in the Essex County area. The role involves evaluating and treating patients in their homes with a schedule from 8:00 am to 4:00 pm, including one weekend every four to six weeks.

The ideal candidate holds a NJ Physical Therapy license, has at least 1 year of home-care experience, and excels in interdisciplinary collaboration while delivering

Qualifications

• HS Diploma or GED is required.
• Bachelor's in Physical Therapy is required.
• Master or Doctorate in Physical Therapy preferred.
• 1 year of homecare experience is strongly preferred.
• NJ License in Physical Therapy is required.
• BLS is preferred.
• Valid and unrestricted Driver License with a clean driving record is required.

Responsibilities

• Evaluate and treat patients in their homes.
• Provide and prescribes plans of care with corrective therapeutic exercise.
• Perform manual treatments including soft tissue techniques and joint mobilizations.
• Supervise and interacts with support staff and maintain proper documentation.
• Evaluate and establish a plan of care and treat patients accordingly.
• Document patient care and charges, including progress and ongoing need for treatment.
• Collaborate with other professionals in an interdisciplinary manner.
• Provide clinically appropriate therapeutic interventions.
• Evaluate patient response to treatment and provide feedback to the team.
• Perform other related duties as assigned.
