Atlantic Health System's Atlantic Visiting Nurse Division is seeking a dynamic Full Time Physical Therapist to evaluate and treat patients in the home setting in Essex County, working with a collaborative team of health professionals.

The schedule is flexible between Monday through Friday, 8:00 am–4:00 pm, with weekend rotation every 5 to 7 weeks. This role supports patient independence and coordinated care across providers.

Competenze

• Licensed physical therapist (DPT) eligible or licensed in NJ.
• Experience with home health evaluation and treatment is a plus.

Mansioni

• Performs initial examinations and differential diagnosis.
• Provides and prescribes plans of care consisting of corrective therapeutic exercise.
• Performs manual treatments consisting of soft tissue techniques, joint mobilizations, and many more.
• Supervises and interacts with support staff and maintain proper documentation.
• Evaluates and establishes a plan of care and treats patients accordingly.
• Documents patient care and charges appropriately, and documents the course of patient care, including progress made and the ongoing need for treatment.
• Works with other professionals in an effective manner
• Provides clinically appropriate therapeutic interventions.
• Evaluates patient response to treatment and provides feedback to the interdisciplinary team.
• Performs other related duties as assigned.
