Job summary

BAYADA Home Health Care in North Carolina is seeking a full-time Physical Therapist to provide in-home visits in Granville and Person counties. You will join a multidisciplinary team delivering rehabilitative care to adult and geriatric clients, enabling them to stay safely at home.

Requirements include an active NC PT license; new grads are encouraged to apply. Strong communication, time management, and EMR skills are preferred.

Qualifications

• Active North Carolina PT license.
• New grads encouraged to apply.
• Graduate from an APTA-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficient computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated territories.
• Perform mobility, range of motion, strength, balance assessments.
• Implement wound care interventions using evidence-based dressings.
• Develop and implement individualized care plans including manual therapy and gait training.
• Collaborate with nursing in the multidisciplinary care plan.
• Educate clients and families on rehabilitation activities.
• Accurately document observations and interventions using EMR tablet.
