BAYADA Home Health Care, Inc. in Roxboro, NC is seeking a Home Health Physical Therapist to perform adult and geriatric home visits. You will evaluate, treat, and educate patients, coordinate with a multidisciplinary team, and document care to support patients staying at home.

The role offers a $5,000 sign-on bonus, comprehensive benefits, and a full-time schedule serving Person, Caswell, and Orange counties.

Formación

• Active North Carolina Physical Therapist license.
• Graduation from an APTA-accredited program or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsabilidades

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, gait, strength, balance, safety).
• Develop and implement individualized care plans with therapeutic exercises and gait training.
• Continuously assess and revise PT care plans in coordination with nursing.
