Job summary

BAYADA Home Health Care, Inc. is seeking a Physical Therapist for full-time home health visits in Wilmington and surrounding towns (Rowley, Ipswich, Newburyport). A valid PT license and at least one year of PT experience are required.

You will assess, treat, educate, and document care as part of a multidisciplinary team, with opportunities for growth. BAYADA offers competitive benefits and a sign-on bonus up to $10,000.

Qualifications

• A current MAP Physical Therapist - PT license.
• Minimum of one year of recent PT experience.
• Graduation from an APTA-approved program or CMAA/APTA accreditation.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements relevant to PT.
• Develop and implement individualized care plans with therapeutic exercises.
• Revise care plans in coordination with nursing and the care team.
• Educate clients and families on rehabilitative care and safety.
• Accurately document observations and interventions in the EMR.
