Job summary

LiveWell Homecare Agency, LLC is seeking a Physical Therapist to provide expert home health services, assess patients, develop and modify care plans, and ensure documentation compliance.

You will collaborate with the healthcare team, educate patients and caregivers, supervise PTA and HHA, and perform OASIS assessments where permitted.

A licensed PT with strong communication and organization skills is preferred for delivering compassionate, compliant care in patients' homes.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Unrestricted state license as a Physical Therapist.
• Proficiency in assessments, documentation, and compliance.
• Preferred: strong communication, organization, attention to detail, and experience in home health and OASIS documentation.

Responsibilities

• Conduct initial and ongoing evaluations of mobility, strength, and function.
• Develop, implement, and modify individualized treatment plans.
• Document treatment progress and discharge plans effectively.
• Coordinate care with healthcare team members and educate patients/caregivers.
