Job summary

Diamond Care Group is seeking a skilled Home Health Physical Therapist (PT) to join our team in Orlando, FL. You will provide individualized physical therapy services to patients in their homes, helping them improve mobility and regain independence after illness or surgery.

Responsibilities include evaluating patients, creating treatment plans, guiding exercises, and educating families. A current PT license, CPR certification, and reliable transportation are required, with 1–2 years in home

Qualifications

• Current Physical Therapist license is required.
• Graduate of accredited PT program.
• 1–2 years of experience preferred in Home Health.
• Strong communication skills.
• Ability to work independently.
• CPR certification current.
• Valid driver’s license and reliable transportation.
• Knowledge of OASIS.
• Experience with EMR systems such as Kinnser.

Responsibilities

• Perform initial evaluations and develop individualized treatment plans.
• Provide therapeutic exercises, manual therapy, and mobility training.
• Assess patient progress and adjust care plans accordingly.
• Educate patients and caregivers on safety and exercises.
• Maintain timely and accurate documentation.
• Collaborate with interdisciplinary care teams.
• Follow home health and Medicare guidelines.
