Job summary

Senior Care Solutions Home Health seeks a reliable Home Health Physical Therapist (PT) to provide in-home therapy across Central Florida. The PT will focus on restoring function, mobility, and independence following illness or surgery while working with an interdisciplinary team.

The ideal candidate has a Florida PT license, CPR certification, and at least 1 year of home health experience, plus strong organizational and communication skills.

Qualifications

• Active Florida PT license required.
• Current CPR certification.
• Minimum 1 year of PT experience, home health preferred.
• Valid driver’s license with reliable transportation.
• Strong organization and communication skills.
• Knowledge of Medicare documentation requirements.

Responsibilities

• Evaluate patients’ functional status and create individualized care plans.
• Deliver therapy services per physician orders and Medicare guidelines.
• Educate patients and caregivers on safe movement, exercises, and home programs.
• Communicate with physicians, nurses, and the care team.
• Accurately document visits and patient progress per standards.
• Ensure continuity of care and timely documentation.
