Job summary

Diamond Care Group seeks a Home Health Physical Therapist to provide individualized PT services in patients' homes, improving mobility and independence after illness, injury, or surgery.

This role includes evaluating patients, creating treatment plans, guiding exercises, and collaborating with interdisciplinary teams, while ensuring compliance with home health and Medicare guidelines.

Qualifications

• Licensed PT in Florida.
• Graduate of an accredited PT program.
• 1-2 years of experience in Home Health preferred.
• Strong communication skills.
• Ability to work independently.
• CPR certification current.
• Valid driver's license and reliable transportation.
• Knowledge of OASIS.
• Experience with EMR systems such as Kinnser.
• Willingness to undergo Level 2 background screening per Florida AHCA.

Responsibilities

• Perform initial evaluations and develop individualized treatment plans.
• Provide therapeutic exercises, manual therapy, and mobility training.
• Assess patient progress and adjust care plans accordingly.
• Educate patients and caregivers on safety and exercises.
• Maintain timely and accurate documentation.
• Collaborate with interdisciplinary care teams.
• Follow home health and Medicare guidelines.
