Job summary

Diamond Care Group is seeking a skilled Home Health Physical Therapist (PT) to join our team. You will provide individualized physical therapy services to patients in their homes, helping them regain mobility after illness or surgery.

The role includes initial evaluations, treatment planning, therapeutic exercises, manual therapy, and coordination with interdisciplinary care teams. Florida PT license and CPR certification are required; 1–2 years of home health experience preferred; valid

Qualifications

• Current Physical Therapist license in Florida.
• Graduate of an accredited PT program.
• 1–2 years of home health experience preferred.
• Strong communication skills.
• Ability to work independently.
• CPR certification currently maintained.
• Valid driver’s license and reliable transportation.
• Knowledge of OASIS.
• Experience with EMR systems such as Kinnser.

Responsibilities

• Perform initial evaluations and develop individualized treatment plans.
• Provide therapeutic exercises, manual therapy, and mobility training.
• Assess patient progress and adjust care plans accordingly.
• Educate patients and caregivers on safety and exercises.
• Maintain timely and accurate documentation.
• Collaborate with interdisciplinary care teams.
• Follow home health and Medicare guidelines.
