Job summary

VitalCaring Group in Austin, TX is seeking a Home Health Physical Therapist to deliver patient-centered care in the home setting, promoting independence and mobility. You will work autonomously within the care team to assess needs and develop plans.

You’ll educate patients and caregivers, document progress in the EMR, supervise PTAs under state guidelines, and collaborate with nurses and physicians to ensure coordinated care. A valid PT license and CPR are required.

Qualifications

• Graduate from an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience preferred.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate and treat patients to improve mobility and independence.
• Develop individualized home health care plans with the care team.
• Educate patients and caregivers on exercises and safety.
• Document patient progress in EMR and participate in case conferences.
• Supervise Physical Therapist Assistants (PTAs) as per state guidelines.
• Collaborate with nurses, therapists, and physicians for coordinated care.
