Job summary

MUSC Health at Home by BAYADA is seeking a Physical Therapist to provide home health visits across Florence/ Marion Counties. You will be part of a multidisciplinary team delivering rehabilitative care and skilled nursing so patients can remain at home.

The role requires a current SC PT license, at least one year of PT experience, and proficiency with EMR systems. You will educate patients and families, assess care needs, and document interventions using a tablet-based system.

Qualifications

• Must hold a current SC Physical Therapist license.
• Minimum of one year of recent work experience as a Physical Therapist.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as mobility, range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Implement wound care and protective interventions to prevent wound formation and promote repair using evidence-based dressings and topical agents.
• Develop and implement individualized care plans, including manual therapy, gait training, balance exercises, and other interventions.
• Continually assess and revise the PT care plan and participate with nursing in the multidisciplinary care plan as appropriate.
• Educate and instruct clients, family members, or other client representatives in rehabilitative care and activities necessary for health, safety, and independence.
• Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.
