MUSC Health at Home by BAYADA

Immediate opening for a Per Diem Physical Therapist, Home Health to provide home health visits for our MUSC Health at Home by BAYADA Office in Summerville, SC and surrounding areas.

As a home health care PT, you will be an integral member of a multi‑disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to remain at home.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements such as mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence‑based dressings and topical agents.
• Develop and implement individualized care plans including manual therapeutic exercises, gait training, balance, and other interventions.
• Continuously assess and revise the PT care plan, and collaborate with nursing in the multidisciplinary care plan as appropriate.
• Educate and instruct clients, family members, or other client representatives on rehabilitative care and activities needed to promote client health, safety, and independent living.
• Accurately document observations, interventions, and evaluations using a state‑of‑the‑art touch‑pad tablet.

Qualifications

• Current SC Physical Therapist license.
• Minimum one year of recent work experience as a Physical Therapist.
• Graduated from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, evidenced by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem‑solving skills.
• Solid computer skills; prior EMR experience preferred.
