Job summary

Star Light Home Health in Montrose/Grand Junction, CO seeks a dedicated Physical Therapist to provide one-on-one home health care. You will perform evaluations, design treatments, and collaborate with a multidisciplinary team to optimize patient outcomes in the home setting.

Must hold a Colorado PT license and be willing to travel to patient homes; OASIS experience is preferred and independent work is common in this role.

Qualifications

• Active Colorado PT license in good standing.
• Graduate of an accredited Physical Therapy program (DPT or equivalent).
• Experience in home health, geriatrics, or Medicare-certified settings preferred.
• Familiarity with OASIS documentation and Medicare home health regulations.
• Valid driver’s license and ability to travel to patient homes in the Grand Junction area.
• Strong communication, time management and critical thinking skills.
• Compassionate, self-motivated and able to work independently.

Responsibilities

• Conduct comprehensive PT evaluations and develop home health treatment plans.
• Deliver skilled PT services including exercise, gait training, balance and pain management.
• Monitor and document patient progress per Medicare guidelines.
• Collaborate with nurses, OTs, and physicians.
• Educate patients and caregivers on home exercise programs and safety.
• Complete OASIS assessments and maintain timely documentation.
• Participate in care coordination and discharge planning.
