Job summary

The University of Vermont Health Network's Adult Home Care is seeking a Physical Therapist for per diem, home-based care across Chittenden & Grand Isle Counties. The role emphasizes comprehensive evaluations, personalized treatment plans, and interventions to improve mobility, strength, and function in the patient's home.

Join a collaborative interdisciplinary team with RNs, LPNs, LNAs, OT, SLP, Social Workers, and staff.

Qualifications

• Bachelor's degree or higher in Physical Therapy.
• 2+ years of general PT experience, or 1 year at PT I level within UVM Health Home Health & Hospice with strong performance.
• Home health experience preferred, but not required, we can train the right person!
• Current Vermont Physical Therapist license, or willingness to obtain one.

Responsibilities

• Perform comprehensive evaluations and assessments.
• Develop individualized treatment plans tailored to each patient's needs.
• Deliver skilled therapy interventions to improve mobility, strength, and function.
• Support patients in remaining in their homes safely and independently.
