Job summary
Senior Care Solutions is seeking a Home Health Physical Therapist Assistant (PTA) to provide skilled PT services in patients' homes under a supervising PT. You will work independently in the field with strong clinical support from management and office staff.
The role requires a Florida PTA license, CPR certification, and 1 year Medicare home health experience is preferred. Excellent communication and documentation skills, plus reliable transportation, are essential.
Qualifications
- Florida PTA license required.
- Minimum 1 year Medicare home health experience preferred.
- Current CPR certification is required.
- Valid driver's license and reliable transportation.
- Strong communication, documentation and time-management skills.
- Ability to work independently under supervising PT’s plan of care.
Responsibilities
- Provide skilled physical therapy treatments per plan of care.
- Carry out therapeutic exercises, gait and balance training.
- Monitor and document patient progress and response to treatment.
- Complete visit documentation in the EMR in a timely manner.
- Communicate patient status to supervising Physical Therapist.
- Educate patients and caregivers on home exercise programs and safety.
- Collaborate with nurses and interdisciplinary team to support goals.
- Adhere to Medicare, ACHC, and agency policies.
