Job summary

Elara Caring is seeking a Physical Therapist to provide in-home evaluations and therapy across the Connecticut Shoreline, including East Haven, Branford, Guilford, and Madison. Join a supportive, clinical team that values one-on-one patient care and flexible scheduling.

As a licensed PT in Connecticut, you will design personalized treatment plans, monitor progress, educate patients and caregivers, and document care with AI-assisted tools.

Qualifications

• Graduate of an American Physical Therapy Association-approved physical therapy program.
• Current and unrestricted Connecticut Physical Therapist license
• Strong communication, organization, and patient-care skills
• Ability to work independently in a home-based environment
• Willingness to travel throughout the assigned territory
• Must possess a valid driver's license, maintain an acceptable driving record or have reliable transportation
• Ability to frequently lift, push, pull, and support up to 50 pounds

Responsibilities

• Evaluate patients’ mobility, strength, balance, safety, and functional needs.
• Develop individualized treatment plans with practical, patient-centered goals.
• Provide skilled physical therapy in patients’ homes.
• Help patients move safely and perform everyday activities with greater independence.
• Monitor progress and adjust treatment plans as needed.
• Educate patients and caregivers on exercises, safe mobility, and techniques to support recovery.
• Collaborate with physicians, nurses, caregivers, and other therapy professionals.
• Complete accurate and timely clinical documentation.
