We value experienced clinicians who can deliver high-quality home-based care and collaborate with a multidisciplinary team to support patients in their homes. Travel up to 50% is required in the branch area.

Formación

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one (1) year of experience in a healthcare facility.
• Able and willing to travel within branch/office coverage area, up to 50%.

Responsabilidades

• Provides direct patient care, evaluates functional needs and outcomes, and adjusts care plans for optimal patient outcomes.
• Designs and implements a plan of care based on thorough assessment.
• Promotes quality of care and directs staff to achieve patient goals.
• Documents evaluations, treatment goals, and plans; maintains records and signatures.
• Travels to patient homes within coverage area and collaborates with the care team.
