Interim HealthCare in Great Bend, KS is seeking a Home Health Physical Therapist to deliver patient-centered care in the home setting. You’ll assess mobility, develop customized plans, and guide patients toward regained function with hands-on therapy and safe activities.

Qualifications

• Graduate of an accredited Physical Therapist Program and active KS PT license.
• CPR certification is required.
• Excellent interpersonal skills, dexterity and compassion are essential.

Responsibilities

• Provide home-based physical therapy to patients with mobility, strength, and functional goals.
• Collaborate with a home health team (RN, LPN, CNA, HHA, OT, SLP) aligned to the care plan.
• Observe movements, conduct assessments, document progress and modify care plan as needed.
• Assist patient with exercises and hands-on therapy to improve mobility and manage pain.
• Educate patients on their plan of care, goals, exercises and self-care.
• Coach family on the plan of care, therapy goals and safe assistance with exercises.
