Interim HealthCare of Central MT is seeking a Home Health Physical Therapist to deliver patient-centered care in the home setting, ensuring mobility and independence for clients in Montana. You will collaborate with a home health team to implement plans of care and monitor progress.

The role emphasizes education for patients and families, adherence to safety protocols, and flexible assignments that support work-life balance while delivering high-quality home health services.

Qualifikationen

• Graduate of an accredited Physical Therapist program with MT licensure.
• Two years of PT experience, preferably in home health.
• CPR certification
• Hep B and TB immunizations
• Knowledge of state and federal home health regulations
• Good clinical judgement, strong interpersonal skills, dexterity and compassion

Aufgaben

• Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals
• Educate patients on their plan of care, established goals, exercises and self-care
• Coach family on the plan of care, therapy goals and safely assisting with exercises
