Job summary

Interim HealthCare of Central MT is seeking a licensed Physical Therapist to deliver home-based therapy across Montana, improving mobility and independence for patients in their homes. You will collaborate with a multidisciplinary home health team to align with the patient’s goals and plan of care.

You will educate patients and families, monitor progress, and adjust plans to maximize outcomes, while adhering to federal and state home health regulations.

Qualifications

• Graduate of an accredited Physical Therapist Program and active PT license in Montana.
• Minimum of two (2) years of physical therapy experience, ideally in home healthcare.
• CPR certification.
• Hep B and TB immunizations.
• Knowledge of state and federal home health regulations.
• Good clinical judgement, strong interpersonal skills, dexterity and compassion.

Responsibilities

• Provide home-based physical therapy to patients with mobility, strength, and functional challenges.
• Work with a home health team including RN, LPN, CNA, HHA, OT and SLP on the plan of care.
• Educate patients on their plan of care, goals, exercises and self-care.
• Coach family on the plan of care, therapy goals and safely assisting with exercises.
