Job summary

Interim HealthCare Inc. in Great Bend, KS is seeking a Home Health Physical Therapist to provide patient-centered, home-based therapy. You will work with a multidisciplinary team to implement individualized plans of care and help patients regain mobility and independence.

We offer 1:1 patient ratios, flexible assignments, access to online training and opportunities to earn CEUs, plus tuition discounts through Rasmussen University. Active Kansas PT license and CPR certification are required.

Qualifications

• Graduated from an accredited Physical Therapist program.
• Active Physical Therapist license in Kansas.
• CPR certification is required.

Responsibilities

• Provide home-based physical therapy to patients with reduced mobility or strength.
• Collaborate with home health team (RN, LPN, CNA, HHA, OT, SLP) on care plans.
• Observe movements, assess progress, and adjust the care plan as needed.
• Assist patients with exercises and hands-on therapy to improve mobility and reduce pain.
• Educate patients on the plan of care, goals, and exercises.
• Coach families on safely assisting with exercises and therapy goals.
