Job summary

Granite Peak Home Health is seeking a qualified Physical Therapist to provide physician-prescribed therapy under a plan of care established by the PT, in a home health setting. You will report to the Home Health Director and collaborate with the care team to return patients to optimum function.

Preferred qualifications include state licensure, two years' experience, and knowledge of HIPAA and healthcare regulations.

Qualifications

• Graduate from a two-year college-level program approved by the APTA.
• Currently licensed or certified in the practicing state.
• Two years experience preferred.
• Ability to exercise initiative and independent judgment.
• Adherence to agency policies and procedures.
• HIPAA compliance knowledge is beneficial.

Responsibilities

• Provides physician-prescribed PT under a PT-planned care.
• Improves residual disabilities and returns patient to function.
• Collaborates with health care team on care planning.
• Performs skilled procedures per PT and plan of care.
• Instructs patients and families in home programs.
• Maintains progress notes and updates care plans promptly.
