Job summary

Adoration Health in Fredericksburg, VA seeks a Home Health Physical Therapist to provide 1:1 patient care in the home setting. You will perform evaluations, develop treatment plans, and coordinate with families and the care team to support recovery and independence.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• New graduates may be considered in select markets.
• At least 2 years of experience as a physical therapist preferred.
• Valid license in the state of practice and CPR certification.

Responsibilities

• Perform PT evaluation and treatment based on physician orders.
• Establish goals and treatment plans to maximize independence.
• Maintain communication with the treatment team, patient, and family.
• Document services promptly for proper billing.
• Supervise assistants and aides per state practice act.
