Adoration Health is seeking a compassionate Home Health Physical Therapist to join our Winston-Salem team. You will provide one-on-one, patient-centered PT services in patients' homes, following physician orders and developing individualized plans to maximize independence.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• CPR certification required; valid state license.
• Minimum 2 years of PT experience; hospice/community experience preferred.

Responsibilities

• Perform physical therapy evaluation and treatment based on physician orders and procure adaptive equipment.
• Establish measurable goals and develop treatment plans to maximize independence.
• Maintain communication among treatment team, physician, patient, and family.
