Job summary

Lifespan Therapy in California is seeking an experienced Home Health Physical Therapist to deliver patient-centered care to geriatric patients in home settings. You’ll evaluate, treat, and develop individualized plans of care, focusing on mobility, balance, and independence.

You’ll enjoy autonomy with your own schedule, use TherapySync EMR for streamlined documentation, and collaborate with a supportive care team to optimize outcomes.

Qualifications

• Valid California Physical Therapist license.
• CPR or BLS certification.
• Driver's license and auto insurance.
• Physical + TB test (within the last year).
• Professional liability and business license (if applicable).

Responsibilities

• Provide Skilled PT Care: Conduct comprehensive in-home evaluations and develop individualized plans of care for geriatric patients.
• Implement Therapeutic Interventions: Focus on functional strengthening, gait training, balance, and fall risk reduction.
• Manage Your Caseload: Schedule and conduct home visits based on your availability and patient needs.
• Collaborate for Optimal Outcomes: Work with a supportive care team to ensure coordinated patient care.
